This Privacy Statement discloses our privacy practices and uses of the information that we gather via scanslated.com, direct.scanslated.com, patient.scanslated.com, any other subdomains, and the Scanslated user-friendly medical report (the “Site”). Your use of the Site means you agree to the terms in this Privacy Statement and our Terms of Use. We encourage you to refer to this Statement periodically so that you understand our current privacy practices. We will not notify you of any updates to this notice.
Information Collected
PatientFriendly collects information from the following sources: by your voluntary submissions, from your medical provider, and through cookie and other online tracking technology. Your medical provider is a hospital, outpatient imaging center, radiology group, or service provider acting on behalf of your medical provider. Scanslated collects the following information:
Personal Information (Identifiers)
We collect your name, phone number, email, and identifying number (“Personal Information”). We need this information to provide our user-friendly medical report.
Characteristics
We collect your age to deliver user-friendly content, for research purposes, and in conjunction with User Generated Content.
Customer Records
We collect medical reports to provide our user-friendly medical report service. Your medical provider will provide the medical report for Scanslated to deliver the user-friendly report. You may also directly upload your medical report to Scanslated. When connected with your Personal Information, this data becomes electronic Protected Health Information (PHI).
User Generated Content
We collect your voluntarily provided answers to survey questions, (“User Generated Content”). Scanslated will disclose your User Generated Content with others without identifying you. If you do not want information disclosed with others, you should not submit User Generated Content. Once you have submitted User Generated Content, we reserve the right to publish it in any medium to others.
Internet Activity
We collect statistical information about how many users view their user-friendly medical report, how long they stay on that page, and which interactive definitions, diagrams, and other content they click on. This information allows us to improve the user experience of and promote our user-friendly medical report. We collect this information through the use of cookies and other tracking technologies, which are discussed in greater detail below.
Cookies
We automatically collect information and data through the use of cookies. Cookies are small text files Scanslated uses to recognize repeat users. You may set your browsers to notify you if you receive a cookie, or you may choose to block cookies with your browser. However, please be aware that some features of our Site may not function or may be slower if you refuse cookies.
Other Tracking Technologies
Tracking technologies record Internet domain and host names; Internet protocol (IP) addresses; browser software and operating system types; clickstream patterns; and dates and times that our Site is accessed. An IP address is a number that is automatically assigned to your computer whenever you are surfing the web. Web servers, the computers that ‘serve up’ webpages, automatically identify your computer by its IP address.
Payment Information
We use a third party payment processing service to collect payment information when you directly upload your medical report.
Sensitive Personal Information
We collect your medical report. We cannot provide our user-friendly medical report without your medical report.
Do Not Track and Global Privacy Control
Do Not Track (DNT) is a privacy preference that users can set in some web browsers, allowing users to opt out of tracking by websites and online services. Scanslated does not engage in any cross context behavioral advertising. For more information on Do Not Track please visit allaboutdnt.com.
Global Privacy Control is a privacy preference that users can set in some web browsers, allowing users to opt out of selling data. Scanslated does not sell user data. For more information on Global Privacy Control please visit globalprivacycontrol.org.
Use of Information
The information Scanslated collects is used:
- to improve the Site and our user-friendly medical report;
- for conducting quality assessments;
- for research activities;
- to let you know about medical services from your medical provider; and
- internal business analysis or other business purposes consistent with our mission.
We store and maintain data for the length of time directed by your medical provider or at your request, and we will delete information as requested by your medical provider or at your request. We will de-identify the information before deleting if allowed by your medical provider, which you may also opt-out.
Disclosures and Transfers of Information
We disclose Personal Information to third parties under the following conditions:
- We have your permission to make the disclosure;
- The disclosure is to your medical provider or to a service provider designated by your medical provider;
- The disclosure is reasonably related to the sale or other disposition of all or part of our business or assets;
- The party to whom the disclosure is made controls, is controlled by, or is under common control with Scanslated;
- The disclosure is in our sole discretion necessary for the establishment or maintenance of legal claims or legal compliance, to satisfy any law, regulation, subpoena or government request, or in connection with litigation; or

The disclosure is to outside businesses to perform certain services for us. The categories of recipients include:
- hosting our Site in a cloud environment
- error detection and notification
- payment processing services
- text, email, and mail message delivery
- customer service phone and email platforms
- document platform for product improvement (“Administrative Service Providers”).
Prior to sharing your Personal Information with others, agreements are made between Scanslated and such outside parties to whom we are transferring data. If Scanslated becomes aware that any such outside party is improperly using or disclosing Personal Information, it will notify that outside party and request that it take reasonable steps to cease such use or disclosure.
Scanslated does not sell or share your Personal Information to third parties. Share means disclosing Personal Information to third parties for cross-context behavioral advertising. We may disclose Personal Information to your medical provider for their compliance, healthcare, and marketing purposes. If you properly notify us that you no longer want us to disclose your Personal Information to your medical provider, we will cease disclosing your Personal Information to your medical provider. However, except as otherwise stated in this Privacy Statement, we do not control your medical provider and you will need to further notify your medical provider directly if you would like them to cease use of your Personal Information.
We may also disclose aggregate visitor data and user generated data in order to describe the use of the Site to our existing or potential business partners or other third parties, or in response to a government request.
Choice: Opt-in/Opt-out
You can also ask us whether we are keeping information about you; how your information is being used; for a copy of the data collected, to update or delete your information, to opt-out of the collection and use of certain information, or to opt-out of collection of sensitive personal information by sending an email to the info@friendlyreports.com, specifying the nature and scope of your request. If you choose to delete your information or opt-out of the collection and use of your information, you understand that certain services may no longer be available to you. Please state in your correspondence the details of your request. We may require proof of your identity before we provide you with the requested information. We allow you to challenge the data that we hold about you and, where appropriate, you may have the data erased, rectified, amended, or completed. Approved requests will be processed within forty-five (45) days of the date on which we receive the request.
Data Security, Integrity, and Access
The Personal Information you provide and we collect is stored through our Administrative Service Providers. We use security measures consistent with industry standards. We have in place technical and organizational security measures, such as data encryption. However, we cannot guarantee the complete security of our databases, nor can we guarantee that information you supply will not be intercepted while being transmitted to us over the Internet.
Your information may be transferred to, and maintained on, computers located outside of your state, province, or country where the privacy laws may not be as protective as those where you live. If you are located outside the United States and choose to provide information to us, please be aware that we transfer Personal Information to the United States and process and store it there. By submitting your Personal Information, you consent to having your personal data transferred to, processed, and stored in the United States.
Scanslated may assign you a user ID and a password as part of your participation and access to the Site. Your user ID and password may only be used by you. You may not share your user ID and password with anyone else, and you are solely responsible for maintaining and protecting the confidentiality of your user ID and password. You are fully responsible for all activities that occur under your user ID and password.
Third Party Websites and Links
Please note that you may have cookies placed on your computer by third party websites that refer you to the Site. Please review the privacy policies of each website you visit to better understand their privacy practices.
In addition, Scanslated would like to inform you that anytime you click on links, which take you to third party websites, Scanslated does not control such other sites and is not responsible for their content, their privacy policies, or their use of Personal Information. Scanslated’s inclusion of such links does not, by itself, imply any endorsement of the content on such sites or of their owners or operators except as disclosed on the Site. Any information submitted by you directly to these third parties is subject to that third party’s privacy policy. Scanslated expressly disclaims any and all liability for the actions of third parties, including but without limitation to actions relating to the use and/or disclosure of Personal Information by third parties.
Children’s Privacy
Scanslated does not intend the Site to be used by individuals under the age of 18 without the supervision of a parent or guardian 18 years of age or older. Scanslated relies upon parents or guardians 18 years of age or older to determine if the Site is appropriate for the viewing, access, or participation by such individuals under the age of 18. If an individual under the age of 18 uses the Site, such individual may only use the Site under the supervision of a parent or guardian 18 years of age or older.
Protected Health Information
We are required by law to maintain the privacy of your protected health information (“PHI”) and to notify you in the event of a breach of your unsecured PHI.
Your Rights
If you are a United States resident, you have the following rights:
- The right to request, up to 2 times in a 12-month period, that we identify to you (1) the categories of personal information we have collected or disclosed about you in the last 12 months, (2) the categories of sources from which the personal information was collected, (3) the business purpose for which we use this information, and (4) the categories of third parties with whom we share or have shared your personal information in the last 12 months;
- The right to request, up to 2 times in a 12-month period, that we disclose to you, free of charge, the specific pieces of personal information we have collected about you in the last 12 months;
- The right to request, up to 2 times in a 12-month period, that we delete personal information that we collected from you, subject to certain exceptions; and
- The right to designate an authorized agent to submit one of the above requests on your behalf.
- The right to not be discriminated against in receiving different or less favorable pricing, service or financial incentive for exercising any of the above rights.
- The right to correct information about you.
- The right to limit use of sensitive personal information.
Governing Law; Visitors Residing Outside the United States
This Privacy Policy and the privacy practices of Scanslated will be subject exclusively to the laws of Florida and venue in Hillsborough County, without regard to its conflict of law principles. Scanslated makes no representation that this Privacy Policy and such practices comply with the laws of any country outside the United States.
